It was the conditions for the Binding Vow set by Yuji, which helped Sukuna take advantage and take over Megumi’s body.

The conditions for the Binding Vow were that if Yuji required healing, Sukuna would heal him in exchange for taking over his body for one minute whenever he said the keyword,‘Enchain’. Yuji also added that while Sukuna would be in control, he would not hurt or kill any of his friends. Yuji tried to fight it off with Sukuna but was instantly defeated and forced to agree to the vow. But Sukuna being the King of Curses, waited for the right moment to use loophole in this Bindind Vow and use it in his favor.
When Yuji and Megumi met Yorozu, Sukuna immediately chanted‘Enchain’and took control of Yuji’s body. He rips one of Yuji’s fingers and converts it into a Cursed object and force feeds it to Megumi Fushiguro and transfers his consciousness into Megumi. He immediately took over him. Ryomen Sukuna always had a liking for Megumi, and now, with his body, he could inherit the Ten Shadows technique.

Jujutsu KaisenFans Think Yuji Should Be Thank For Sukuna’s Return
Fans were shocked to see Ryomen Sukuna come back, and that too by taking over a fan-favorite character. What the fans feared the most,Jujutsu Kaisenturned it into a reality. But the fans think that it was all Yuji’s fault. Yuji Itadori has always been the most selfless guy inJujutsu Kaisen. Due to this nature, in the Binding Vow, he prohibited Sukuna from hurting his friends but forgot to add himself.

Sukuna ripping off his finger was legit in terms of the Binding Vow since he could hurt Yuji. As a result, Sukuna gets back his body, Megumi is on the verge of death, Gojo is dead, Kashimo Hajime is dead, and the rampage continues. Yuji also understood what a mishap he made, and so he along with Higuruma, is currently fighting Sukuna.
